Lakeland IV + Wellness
Subscription Terms
For all active recurring subscriptions and memberships offered through Lakeland IV + Wellness, a service of Lakeland Birth & Gynecology.
Subscription & Cancellation Terms
All subscription cancellations require at least 7 full calendar days' notice before the next scheduled recurring payment or prescription renewal processing date, whichever occurs first.
Recurring payments and prescription renewals may begin processing before the renewal date. This advance notice allows our office adequate time to stop automatic billing, coordinate with the dispensing pharmacy when applicable, and prevent unnecessary prescription renewals or shipments.
Cancellation requests received fewer than 7 days before the next scheduled renewal may not take effect until the following billing cycle. Payments already processed and prescriptions already submitted to the pharmacy may not be eligible for cancellation or refund, subject to applicable law and the existing refund policy.
Cancellation stops future renewals after the effective cancellation date. Any existing minimum membership commitment remains subject to its agreed terms.
Patients may discontinue a medication for medical reasons at any time. This administrative cancellation policy does not restrict clinical care or medical decision-making.
Existing membership commitments
IV Wellness and NAD+ IV memberships retain their initial three-month commitment. NAD+ injection memberships and the listed PT-141, sermorelin and semaglutide tablet tier subscriptions retain their existing no-minimum terms. Your selected agreement governs other existing commitments, billing schedules, credits and refund eligibility.
Request cancellation
Send a message to our office through your Patient Portal. Include the name of the subscription you wish to cancel and your next scheduled payment or prescription renewal processing date, if known. Please request acknowledgment of receipt.
Message the OfficeCancellation requires staff processing. Keep your submitted message and its date and time for your records. An acknowledgment confirms receipt only, not that billing or a pharmacy renewal has stopped. Our office must review the request, address applicable billing and pharmacy processing, and confirm the effective cancellation date through the portal. Until confirmed, do not assume a scheduled charge or prescription renewal has been canceled.
